Clinical Overview
Esomeprazole (40mg enteric-coated) and Domperidone (30mg sustained-release) combines the pure S-enantiomer of omeprazole with an antiemetic prokinetic. Esomeprazole provides superior bioavailability and acid suppression compared to racemic omeprazole by undergoing lower first-pass hepatic metabolism via CYP2C19. Domperidone provides dopamine D2 receptor antagonism, enhancing lower esophageal sphincter tone and gastroduodenal transit. Indicated for erosive reflux esophagitis, GERD poorly responsive to standard acid suppression, and diabetic gastroparesis with nausea.
