Clinical Overview

Paracetamol (162.5mg/325mg) and Tramadol Hydrochloride (18.75mg/37.5mg) Tablets is a synergistic, multi-mechanistic analgesic combination on the WHO Model List framework for moderate-to-severe pain. Tramadol acts as a weak mu-opioid receptor agonist while simultaneously inhibiting the reuptake of norepinephrine and serotonin (5-HT) in the descending spinal pain inhibitory pathways. Paracetamol acts centrally to inhibit prostaglandin synthesis and activate descending serotonergic pathways. Combining them produces additive analgesia at lower individual doses with faster onset (paracetamol ~30 min) and extended duration (tramadol 6-8 hours). Indicated for management of moderate-to-severe acute pain, postoperative pain, acute musculoskeletal trauma, and severe osteoarthritis flares.

Usage Guidelines

Take orally 1 to 2 tablets every 4 to 6 hours as needed for severe pain with a glass of water. Maximum dose: do NOT exceed 8 tablets daily (300 mg tramadol / 2600 mg paracetamol). Swallow whole; do not crush or chew. Take with food if nausea occurs. Strictly avoid alcohol.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Nausea, dizziness, somnolence, constipation, sweating, headache, dry mouth, vomiting. Rare/Severe: Respiratory depression, seizures (lowers convulsive threshold), serotonin syndrome (with serotonergic drugs), physical dependence/addiction, and acute hepatotoxicity (paracetamol overdose).

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Schedule H1 Prescription Opioid: Risk of physical dependence, tolerance, and misuse. Lowers seizure threshold; contraindicated in patients with epilepsy or taking SSRIs/SNRIs/TCAs. Strictly avoid alcohol (risk of fatal respiratory depression). Contraindicated in severe hepatic or renal impairment.
