Clinical Overview

Paracetamol / Acetaminophen (Tablets 500mg, 650mg, 1000mg; Oral Suspension 120mg, 125mg, 250mg per 5ml; Drops 100mg/ml) is a first-line non-opioid analgesic and antipyretic on the WHO Model List of Essential Medicines. It selectively inhibits central prostaglandin synthesis through peroxidase-dependent inhibition of COX enzymes in the brain and spinal cord, activates descending serotonergic inhibitory pathways, and resets the hypothalamic thermoregulatory center to reduce fever through cutaneous vasodilation and sweating. Unlike NSAIDs, it does NOT inhibit peripheral cyclooxygenase, causing zero gastric mucosal ulceration, platelet dysfunction, or renal vasoconstriction. Indicated as first-line therapy for mild-to-moderate pain (headache, toothache, dysmenorrhea, osteoarthritis) and fever of diverse etiologies.

Usage Guidelines

Adults: Take 500 mg to 1000 mg orally every 4 to 6 hours as needed with water (maximum 4000 mg daily). Children: Dose strictly by weight: 10 to 15 mg/kg per dose every 4 to 6 hours (maximum 60 mg/kg/day or 4 doses in 24 hours). Measure pediatric suspensions/drops with a calibrated syringe or measuring cup. Can be taken with or without food.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Extremely well tolerated at therapeutic doses. Common: Rare mild nausea or rash. Rare/Severe: Acute dose-dependent hepatic necrosis (with doses >7.5-10 g in adults), acute liver failure, and severe cutaneous adverse reactions (Stevens-Johnson syndrome, TEN).

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Over-The-Counter analgesic/antipyretic. Do NOT exceed 4000 mg daily in adults (or 10-15 mg/kg/dose in children); accidental overdose causes fatal, irreversible acute liver failure. Avoid combining multiple OTC cough/cold medicines containing paracetamol. Chronic alcohol users (>3 drinks/day) should not exceed 2000 mg daily. N-acetylcysteine (NAC) is the specific antidote for acute toxicity.
