Clinical Overview

Tamsulosin Hydrochloride (0.4mg modified-release) and Dutasteride (0.5mg) Tablets/Capsules is a synergistic dual-action therapy on the WHO Model List framework for symptomatic Benign Prostatic Hyperplasia (BPH). Tamsulosin is a uroselective alpha-1A/1D adrenergic receptor antagonist that relaxes smooth muscle in the prostate stroma, prostatic capsule, and bladder neck, rapidly improving urinary peak flow rates and relieving lower urinary tract symptoms (LUTS). Dutasteride is a potent dual type 1 and type 2 5-alpha reductase inhibitor (5-ARI) that suppresses dihydrotestosterone (DHT) synthesis by >90%, progressively shrinking enlarged prostate volume over 3-6 months. Indicated for moderate-to-severe BPH with enlarged prostate (volume >30cc) to reduce the risk of acute urinary retention (AUR) and the need for BPH-related surgery (TURP).

Usage Guidelines

Take orally once daily, approximately 30 minutes after the same meal each day (preferably dinner or breakfast) with a full glass of water. Swallow the tablet/capsule whole; do NOT chew, crush, or open capsules, as direct contact with dutasteride can irritate oropharyngeal mucosa. Long-term therapy (at least 6-12 months) is necessary for maximal prostate shrinkage.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Retrograde ejaculation (ejaculatory dysfunction), erectile dysfunction, decreased libido, dizziness, rhinitis, orthostatic hypotension. Rare/Severe: Intraoperative Floppy Iris Syndrome (IFIS) during cataract surgery, gynecomastia, breast tenderness, and severe hypersensitivity/angioedema.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Contraindicated in women and children. Women who are pregnant or may become pregnant must NOT handle broken or leaking dutasteride capsules due to risk of fetal male external genitalia malformation. Inform ophthalmologist before cataract surgery due to IFIS risk. Dutasteride lowers serum PSA by 50% after 6 months; double measured PSA levels when evaluating for prostate cancer.
