Clinical Overview

Chlorhexidine Gluconate Mouthwash (0.2% w/v; with or without Sodium Fluoride 0.05% and Zinc Chloride 0.09%) is an oral broad-spectrum antiseptic and anti-plaque rinse on the WHO Model List of Essential Medicines. As a cationic bisbiguanide, chlorhexidine binds tenaciously to bacterial cell walls and oral pellicles, adsorbing to oral surfaces and releasing slowly over 12 hours (substantivity) to provide sustained bactericidal and bacteriostatic activity against gram-positive and gram-negative pathogens. Indicated for prevention and treatment of gingivitis, dental plaque reduction, aphthous ulcers, post-periodontal surgery hygiene, and prevention of ventilator-associated pneumonia in ICU patients.

Usage Guidelines

Rinse the mouth thoroughly with 10 mL of undiluted mouthwash for 1 minute twice daily after tooth brushing, then spit out completely. Do NOT swallow. Do NOT rinse with water, eat, or drink for at least 30 minutes after use. Toothpaste contains anionic surfactants (sodium lauryl sulfate) that inactivate chlorhexidine; rinse mouth with water between brushing and using chlorhexidine.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Reversible brown staining of teeth, tongue, and restorations; transient taste alteration (dysgeusia); increased calculus (tartar) formation; mild burning sensation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Over-The-Counter oral rinse. For oral rinsing ONLY; do NOT swallow. Prolonged continuous use (>2 weeks) may cause noticeable tooth discoloration which requires professional dental prophylaxis to remove.
