Clinical Overview

Endothelin Receptor Antagonist (ERA) on the WHO Model List of Essential Medicines. Endothelin-1 is a potent endogenous vasoconstrictor and vascular smooth muscle mitogen; ERAs selectively or dualistically block endothelin receptors (ETA and ETB) on pulmonary vascular smooth muscle, reversing pulmonary vasoconstriction, inhibiting vascular remodeling, and reducing pulmonary vascular resistance. Indicated for Pulmonary Arterial Hypertension (PAH, WHO Group 1) to improve exercise capacity and delay clinical worsening.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black box warnings: (1) Severe teratogenicity/embryo-fetal toxicity (monthly pregnancy testing mandatory in females of childbearing potential); (2) Hepatotoxicity (monthly liver enzyme monitoring mandatory for bosentan). Monitor hemoglobin/hematocrit periodically.
