Clinical Overview

Tapentadol Tablets (50mg immediate-release, 100mg extended-release) is a centrally acting synthetic analgesic with a dual mechanism: potent mu-opioid receptor agonism (MOR) and noradrenaline reuptake inhibition (NRI). Unlike tramadol, it has minimal serotonergic activity (virtually eliminating serotonin syndrome risk) and does not rely on metabolic CYP2D6 bioactivation for efficacy, producing consistent and predictable analgesia. Indicated for management of severe acute pain (immediate-release) and severe chronic pain / painful diabetic peripheral neuropathy (extended-release).

Usage Guidelines

IR: 50 mg to 100 mg orally every 4 to 6 hours as needed (maximum 500 mg on day 1, 400 mg daily thereafter). ER: 100 mg orally every 12 hours with water, swallowed whole; do NOT crush, chew, or divide. Take with or without food.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Nausea, dizziness, somnolence, constipation, headache, vomiting, pruritus. Rare/Severe: Life-threatening respiratory depression, severe hypotension, syncope, seizures, and addiction/abuse liability.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Schedule H1 / Controlled Substance: High potential for abuse and respiratory depression; monitor closely. Extended-release tablets must be swallowed whole; crushing releases a fatal opioid bolus. Contraindicated in severe respiratory depression, paralytic ileus, or concomitant MAO inhibitors (within 14 days).
