Clinical Overview
Baclofen Tablets (10mg, 20mg Extended Release Gastric Retentive System) is an oral centrally acting skeletal muscle relaxant and antispasticity agent on the WHO Model List of Essential Medicines. It is a synthetic chemical analogue of gamma-aminobutyric acid (GABA) that selectively activates G-protein-coupled presynaptic GABA-B receptors in the spinal cord. This stimulates hyperpolarization by increasing potassium conductance and inhibits calcium influx, reducing the release of excitatory neurotransmitters (glutamate, aspartate) and dampening mono- and polysynaptic spinal reflexes. Indicated for alleviation of signs and symptoms of spasticity resulting from multiple sclerosis, spinal cord trauma, transverse myelitis, and motor neuron disease.

Usage Guidelines
Take orally with food or milk with a full glass of water. Start at 5 mg three times daily; gradually titrate upward every 3 days by 5 mg per dose up to 40 mg to 80 mg daily in divided doses. Swallow extended-release tablets whole. NEVER discontinue abruptly.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Somnolence, sedation, dizziness, muscular weakness, fatigue, nausea, confusion, headache. Rare/Severe: Severe withdrawal syndrome upon abrupt cessation (auditory/visual hallucinations, delirium, hyperthermia, seizures, and rebound spasticity), respiratory depression, and coma.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Abrupt discontinuation can precipitate life-threatening withdrawal syndrome (hyperpyrexia, seizures, psychotic delirium, rhabdomyolysis, and death); always taper dose gradually over 1 to 2 weeks. Causes marked somnolence; avoid driving or operating machinery. Strictly avoid alcohol. Dose must be reduced in renal impairment.
