Clinical Overview

Racecadotril (100mg Capsules; 10mg, 15mg, 30mg Sachets) is an oral peripheral enkephalinase inhibitor antisecretory antidiarrheal on the WHO Model List framework. It is a prodrug hydrolyzed to thiorphan, which inhibits enkephalinase, sparing endogenous opioid enkephalins in the gut. This selectively reduces intestinal hypersecretion of water and electrolytes induced by cholera toxin, rotavirus, or inflammatory enterotoxins without delaying bowel transit or causing secondary constipation or bacterial overgrowth. Indicated for complementary symptomatic treatment of acute watery diarrhea in adults (capsules) and infants/children >3 months (sachets) alongside Oral Rehydration Therapy (ORS).

Usage Guidelines

Adults: Take one 100 mg capsule orally three times daily before meals with water. Pediatric sachets: Empty the sachet into water, ORS solution, or baby food and mix thoroughly; dose is approximately 1.5 mg/kg three times daily. Continue until two normal stools are passed, not exceeding 7 days. ALWAYS give alongside Oral Rehydration Salts (ORS).

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Headache, mild nausea, constipation (rare). Rare/Severe: Angioedema, erythema multiforme, toxicoderma, and severe cutaneous reactions.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Antisecretory, NOT an antimotility agent. Does not replace fluid rehydration; ORS must always be co-administered. Contraindicated in blood-containing stools (dysentery), antibiotic-associated diarrhea, or infants <3 months.
