Clinical Overview
Prucalopride Tablets (1mg, 2mg) is a first-in-class, high-affinity, selective serotonin 5-HT4 receptor agonist prokinetic. It stimulates 5-HT4 receptors on colonic intramural neurons, triggering giant migrating contractions that accelerate colonic transit without interacting with dopamine D2 receptors, 5-HT1, 5-HT2, or cardiac HERG potassium channels. Indicated for symptomatic treatment of chronic idiopathic constipation in adults in whom conventional laxatives fail to provide adequate relief.

Usage Guidelines
Take orally once daily, 2 mg once daily (1 mg once daily in elderly >65 years or patients with renal impairment CrCl < 50 mL/min) with or without food, with a glass of water. Swallow whole. If no improvement is observed after 4 weeks of therapy, re-evaluate treatment benefit.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Headache, nausea, abdominal pain, diarrhea, dizziness, fatigue (most prominent on day 1 and typically resolve within a few days). Rare/Severe: Palpitations, rectal bleeding, and severe depression/suicidal ideation (rare reported associations).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Prescription prokinetic. Unlike cisapride or tegaserod, prucalopride does NOT block HERG channels and does not induce QT prolongation or ischemic events. Contraindicated in renal failure requiring dialysis, intestinal perforation, obstructive ileus, and severe inflammatory bowel disease (Crohn's, ulcerative colitis).
