Clinical Overview
Pantoprazole (20mg/40mg enteric-coated) and Domperidone (10mg/30mg sustained/prolonged-release) is a fixed-dose combination antiulcer and prokinetic therapy. Pantoprazole irreversibly binds to the gastric H+/K+-ATPase proton pump on parietal cells, suppressing basal and stimulated hydrochloric acid secretion for >24 hours. Domperidone is a peripheral dopamine D2 receptor antagonist that accelerates gastric emptying and increases lower esophageal sphincter (LES) tone without crossing the blood-brain barrier. Indicated for gastroesophageal reflux disease (GERD) with reflux esophagitis, non-ulcer dyspepsia, diabetic gastroparesis, and acid peptic disorders associated with nausea, fullness, or early satiety.

Usage Guidelines
Take orally once daily in the morning, 30 to 60 minutes before breakfast with a glass of water. Swallow the capsule/tablet whole; do NOT crush, chew, or open enteric-coated pellets, as gastric acid rapidly degrades unprotected pantoprazole. Standard treatment duration is 4 to 8 weeks depending on endoscopic findings.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Headache, dry mouth, diarrhea, abdominal discomfort, flatulence, transient dizziness. Rare/Severe: Hyperprolactinemia (galactorrhea, gynecomastia, amenorrhea), QT interval prolongation and ventricular arrhythmias (domperidone dose-related risk), Clostridioides difficile-associated diarrhea, hypomagnesemia, and bone fracture risk with long-term PPI therapy.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Domperidone is contraindicated in patients with underlying cardiac conduction abnormalities (prolonged QTc, hypokalemia), congestive heart failure, severe hepatic impairment, or prolactin-secreting pituitary tumors. PPIs may decrease intestinal absorption of calcium, magnesium, and vitamin B12 upon chronic use. Caution with concomitant CYP3A4 inhibitors (ketoconazole, erythromycin).
