Clinical Overview
Topical Carbonic Anhydrase Inhibitor (Dorzolamide 2% or Brinzolamide 1% suspension, alone or with Timolol) on the WHO Model List of Essential Medicines. It inhibits carbonic anhydrase II (CA-II) in the ciliary processes, slowing bicarbonate ion formation and reducing active sodium and fluid transport, thereby diminishing aqueous humor secretion by up to 20-30%. Indicated for intraocular pressure reduction in open-angle glaucoma and ocular hypertension.

Usage Guidelines
Shake suspension (brinzolamide) well before use. Instill one drop into the affected eye(s) two or three times daily. Perform nasolacrimal occlusion. Wait 10 minutes between different eye drops.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Bitter/sour taste in mouth (dysgeusia, via nasolacrimal drainage), burning, stinging, transient blurred vision, punctate keratitis. Rare/Severe: Sulfonamide hypersensitivity (angioedema, Stevens-Johnson syndrome), corneal decompensation, and aplastic anemia.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Sulfonamide derivative: contraindicated in patients with known hypersensitivity to sulfonamides. Caution in patients with severe renal impairment (CrCl < 30 mL/min) or low endothelial cell counts (risk of irreversible corneal edema).
