Clinical Overview
Timolol Maleate (0.5% w/v) is a non-selective beta-1 and beta-2 adrenergic receptor antagonist on the WHO Model List of Essential Medicines. It blocks beta-adrenergic receptors on non-pigmented ciliary epithelial cells, inhibiting cyclic AMP synthesis and reducing aqueous humor secretion by 30-50% without altering pupillary size or accommodation. Indicated for lowering elevated intraocular pressure in patients with chronic open-angle glaucoma and ocular hypertension.

Usage Guidelines
Instill one drop into the affected eye(s) once or twice daily. Apply gentle finger pressure to the inner corner of the eye (nasolacrimal occlusion) for 2 minutes immediately after instillation to minimize systemic cardiovascular and pulmonary absorption.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Transient eye burning, stinging, dry eyes, blurred vision. Rare/Severe: Systemic beta-blocker absorption: severe bradycardia, heart block, hypotension, syncope, bronchospasm, and exacerbation of congestive heart failure.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black box caution for systemic beta-blockade: Strictly contraindicated in bronchial asthma, severe COPD, sinus bradycardia, second- or third-degree AV block, and cardiogenic shock. Diabetic patients must be warned that timolol masks hypoglycemic tachycardia.
