Clinical Overview
Artesunate is a water-soluble hemisuccinate derivative of artemisinin and the WHO gold-standard first-line treatment for severe malaria. Its endoperoxide bridge is cleaved by intraparasitic heme-iron, generating free radicals that alkylate essential parasitic proteins and PfATP6, rapidly reducing parasite biomass within hours. Indicated parenterally for severe Plasmodium falciparum and vivax malaria, cerebral malaria, and complicated malarial infections.

Usage Guidelines
Administered strictly by a qualified healthcare professional by slow intravenous injection over 1-2 minutes or deep intramuscular injection after reconstitution with sodium bicarbonate diluent and 5% Dextrose/Normal Saline. Full parenteral course followed by oral ACT as soon as patient can tolerate oral medications.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Dizziness, nausea, vomiting, mild abdominal pain, headache, transient reticulocytopenia. Rare/Severe: Delayed post-artesunate hemolytic anemia (occurring 1-3 weeks after treatment), severe hypersensitivity/anaphylaxis, and cardiotoxicity at excessive doses.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Monitor hemoglobin and hematocrit for up to 4 weeks following treatment to detect delayed post-artesunate hemolysis. Once the patient is stabilized and capable of oral intake, a complete 3-day course of an alternative oral ACT must be completed to prevent recrudescence.
