Magnesium Sulphate Injection (50% w/v) is a parenteral electrolyte and neuroprotective anticonvulsant on the WHO Model List of Essential Medicines. In obstetrics, it blocks NMDA glutamate receptors in the CNS, inhibits acetylcholine release at motor endplates, and produces cerebral vasodilation, suppressing seizures. Indicated as the gold standard for prevention and treatment of eclamptic seizures in pre-eclampsia/eclampsia, acute severe hypomagnesemia, and life-threatening Torsades de Pointes ventricular arrhythmias.

Usage Guidelines
Administer intravenously or deep intramuscularly according to validated Pritchard or Zuspan eclampsia regimens under hospital monitoring: IV loading dose of 4 g diluted over 15 to 20 minutes, followed by maintenance infusion of 1 to 2 g/hour. MANDATORY MONITORING: Before administering maintenance doses, verify that patellar deep tendon reflexes are present, respiratory rate is >= 16 breaths/min, and hourly urine output is >= 30 mL/hr. Calcium gluconate 10% IV MUST be immediately available as the antidote for toxicity.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Flushing, sensation of warmth, diaphoresis, hypotension, mild drowsiness. Rare/Severe: Loss of deep tendon reflexes (first sign of toxicity), respiratory depression, pulmonary edema, and cardiac arrest.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Obstetric Warning: Magnesium toxicity; loss of patellar reflexes occurs at 8-10 mEq/L, respiratory paralysis at 12 mEq/L, and cardiac arrest at >15 mEq/L. Calcium gluconate 10% IV (10 mL) MUST be kept at the bedside as the immediate antidote. Contraindicated in myasthenia gravis and severe heart block.
