Clinical Overview

Flecainide Acetate is a potent Class Ic membrane-stabilizing antiarrhythmic agent. It potently and selectively blocks voltage-gated cardiac fast sodium channels (Nav1.5) with slow binding and unbinding kinetics, markedly depressing phase 0 depolarization and significantly slowing conduction velocity throughout the His-Purkinje system and ventricular myocardium with minimal effect on repolarization. Indicated for paroxysmal supraventricular tachycardias (PSVT), paroxysmal atrial fibrillation/flutter in patients without structural heart disease, and life-threatening sustained ventricular arrhythmias.

Usage Guidelines

Take orally twice daily (every 12 hours) with water, with or without meals. Strictly adhere to exact dosing times. Do not alter dose without electrophysiologist supervision.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dizziness, visual disturbances (blurred vision, diplopia), headache, nausea, asthenia, tremor, dyspnea. Rare/Severe: Boxed warnings: Proarrhythmic events (new or worsened ventricular arrhythmias, ventricular tachycardia, ventricular fibrillation in patients with structural heart disease); cardiac arrest, and heart failure decompensation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black box warning: In the CAST trial, flecainide significantly increased mortality in patients with prior myocardial infarction or structural heart disease; strictly contraindicated in patients with structural heart disease, coronary artery disease, heart failure, or ischemic cardiomyopathy. Regular ECG monitoring (QRS duration widening) is mandatory.
