🧴 Clinical Application
Take orally twice daily (every 12 hours) with water, with or without meals. Strictly adhere to exact dosing times. Do not alter dose without electrophysiologist supervision.
⚠️ Side Effects
Common: Dizziness, visual disturbances (blurred vision, diplopia), headache, nausea, asthenia, tremor, dyspnea. Rare/Severe: Boxed warnings: Proarrhythmic events (new or worsened ventricular arrhythmias, ventricular tachycardia, ventricular fibrillation in patients with structural heart disease); cardiac arrest, and heart failure decompensation.
🛡️ Safety Warnings
Black box warning: In the CAST trial, flecainide significantly increased mortality in patients with prior myocardial infarction or structural heart disease; strictly contraindicated in patients with structural heart disease, coronary artery disease, heart failure, or ischemic cardiomyopathy. Regular ECG monitoring (QRS duration widening) is mandatory.
