Selexipag is an orally active, selective, non-prostanoid prostacyclin (IP) receptor agonist on the WHO Model List of Essential Medicines. It is hydrolyzed by carboxylesterase 1 into an active metabolite with 37-fold higher potency. Stimulation of the IP receptor activates adenylyl cyclase, elevating intracellular cyclic AMP and producing potent pulmonary vasodilation and anti-proliferative effects. Indicated for the long-term treatment of Pulmonary Arterial Hypertension (PAH, WHO Group 1) to delay disease progression and reduce hospitalization.

Usage Guidelines
Take orally twice daily (morning and evening) with food to improve tolerability, with water. Titrate dose gradually up to the highest tolerated individual maintenance dose as instructed by your PAH specialist.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Headache, diarrhea, jaw pain, nausea, myalgia, vomiting, pain in extremity, flushing, arthralgia. Rare/Severe: Severe hypotension, anemia, pulmonary edema in pulmonary veno-occlusive disease, and hyperthyroidism.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
High incidence of prostacyclin-related vasodilatory adverse effects (jaw pain, headache, diarrhea) during dose titration; manage with symptom relief rather than abrupt discontinuation. Contraindicated in severe coronary artery disease and decompensated heart failure.
