Leucovorin Calcium / Folinic Acid Tablets (15mg) is the 5-formyl derivative of tetrahydrofolic acid (FH4) on the WHO Model List of Essential Medicines. It bypasses the enzymatic block created by dihydrofolate reductase (DHFR) inhibitors (such as methotrexate), directly entering folate cofactor pools to rescue normal host tissues from lethal toxicity ("Leucovorin Rescue"). Concurrently, in 5-fluorouracil chemotherapy, leucovorin stabilizes the ternary binding complex with thymidylate synthase, markedly enhancing 5-FU cytotoxicity. Indicated for rescue therapy following high-dose methotrexate chemotherapy, accidental methotrexate overdose, and potentiation of 5-FU in metastatic colorectal carcinoma.

Usage Guidelines
Methotrexate rescue: Administer orally (or IV for doses >25 mg) starting exactly 24 hours after the initiation of high-dose methotrexate infusion, typically 15 mg every 6 hours for 8 to 10 doses, until serum methotrexate falls below 0.05-0.1 micromolar. Take with water.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Extremely well tolerated. Common: Mild nausea, vomiting, allergic rash, insomnia. Negligible systemic toxicity.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Do NOT administer intrathecally (lethal). High-dose methotrexate rescue requires precise timing; starting leucovorin too early blunts methotrexate antitumor efficacy, while delaying past 40 hours can result in irreversible, fatal bone marrow and renal aplasia. Does not treat pernicious anemia (can mask neurological damage).
