Clinical Overview

Entacapone is a reversible, selective, and peripherally acting catechol-O-methyltransferase (COMT) inhibitor. It blocks the conversion of levodopa to 3-O-methyldopa (3-OMD) in peripheral tissues, leading to sustained plasma levodopa concentrations and greater brain bioavailability. Indicated as an adjunct to levodopa and carbidopa for Parkinson's disease patients experiencing end-of-dose motor fluctuations ("wearing-off").

Usage Guidelines

Take orally simultaneously with each dose of levodopa/carbidopa, up to a maximum of 8 times daily (1600 mg/day), with or without food. Swallow whole.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Harmless reddish-brown/orange discoloration of urine, dyskinesia, nausea, diarrhea, abdominal pain, dizziness. Rare/Severe: Severe persistent diarrhea/colitis, rhabdomyolysis, and hallucinations.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Harmless orange-brown urine discoloration occurs in almost all patients. Contraindicated in pheochromocytoma and neuroleptic malignant syndrome history. Discontinue if severe watery diarrhea occurs.
