Clinical Overview

Montelukast Sodium Tablets (5mg, 10mg) is an oral selective and competitive cysteinyl leukotriene receptor-1 (CysLT1) antagonist on the WHO Model List of Essential Medicines. It blocks the binding of LTD4 (and LTC4/LTE4) to CysLT1 receptors on airway smooth muscle cells, bronchial mucosal endothelial cells, and eosinophils, inhibiting leukotriene-mediated bronchoconstriction, mucous hypersecretion, vascular permeability, and eosinophil migration. Indicated for prophylaxis and chronic treatment of bronchial asthma in adults and pediatric patients >=6 months, prevention of exercise-induced bronchoconstriction, and relief of symptoms of allergic rhinitis.

Usage Guidelines

Asthma and/or allergic rhinitis: Take orally once daily in the evening with a glass of water, with or without food (10 mg for adults; 5 mg for children 6-14 years). Exercise-induced bronchospasm: Take 10 mg at least 2 hours before exercise. NOT for reversal of acute bronchospasm.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Upper respiratory tract infection, fever, headache, pharyngitis, cough, abdominal pain, diarrhea. Rare/Severe: Neuropsychiatric events (agitation, aggression, depression, sleep disturbances, suicidal ideation), systemic eosinophilia and Churg-Strauss syndrome, and hepatotoxicity.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black Box Warning: Serious neuropsychiatric events (suicidal thoughts and actions, depression, aggression, sleep disturbances); discontinue immediately and seek medical attention if mood or behavioral changes occur. Not a rescue medication for acute asthma attacks; patients must continue rescue inhalers.
