Clinical Overview
Salmeterol Xinafoate (25mcg/50mcg) and Fluticasone Propionate (100mcg/250mcg/500mcg Inhaler, Rotacaps) is a benchmark inhaled corticosteroid and long-acting beta-2 agonist combination on the WHO Model List of Essential Medicines. Fluticasone exerts profound local anti-inflammatory, immunosuppressive, and anti-edematous effects in bronchial mucosa with low systemic bioavailability due to extensive first-pass metabolism. Salmeterol provides long-acting bronchodilation lasting over 12 hours via persistent beta-2 receptor activation. Indicated for maintenance therapy of persistent bronchial asthma and maintenance treatment of airflow obstruction in patients with severe COPD and recurrent exacerbations.

Usage Guidelines
Administer twice daily (morning and evening, approximately 12 hours apart) by oral inhalation. MDI: Shake well, inhale slowly and deeply, and hold breath for 10 seconds. Rotacaps: Inhale dry powder deeply through the specialized rotahaler device. MANDATORY: Rinse mouth with water and spit out immediately after inhalation. NOT for acute bronchospasm rescue (has slow bronchodilator onset ~10-20 minutes).
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Oropharyngeal candidiasis, hoarseness/dysphonia, throat irritation, headache, palpitations, muscle cramps. Rare/Severe: Paradoxical bronchospasm, pneumonia in COPD patients, adrenal suppression with prolonged high doses, and bone density reduction.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Not for acute asthma attacks; patients must carry a rapid-acting rescue inhaler (salbutamol). Rinse mouth and gargle with water after every inhalation. LABA monotherapy is contraindicated in asthma (must always be paired with an inhaled steroid).
