Clinical Overview
Pentazocine Injection (30mg/ml) is a synthetic opioid agonist-antagonist analgesic on the WHO Model List framework. It acts as an agonist at kappa-opioid receptors (producing spinal analgesia, sedation, and dysphoria) and a weak partial agonist or antagonist at mu-opioid receptors. It increases blood pressure and systemic vascular resistance due to catecholamine release. Indicated for relief of moderate-to-severe pain, preoperative/pre-anesthetic sedation, and supplement to surgical anesthesia.

Usage Guidelines
Administer by intramuscular (IM), subcutaneous (SC), or slow intravenous (IV) injection: 30 mg every 3 to 4 hours as needed (maximum 360 mg daily). When administering IV, inject slowly over 1 to 2 minutes. Rotate injection sites frequently to prevent severe tissue induration.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Sedation, dizziness, nausea, vomiting, euphoria or dysphoria, diaphoresis. Rare/Severe: Respiratory depression, severe tissue fibrosis / cutaneous ulceration at injection sites, hallucinations/psychomimetic effects, hypertension, and precipitation of acute withdrawal in opioid-dependent individuals.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Schedule H1 Controlled Substance: Abuse and dependence liability. Causes severe injection site sclerosis, tissue induration, and ulceration with repetitive subcutaneous injections. Because of intrinsic mu-antagonist properties, it precipitates acute withdrawal in patients dependent on pure mu-opioids (morphine, fentanyl). Increases myocardial work; use caution in acute myocardial infarction.
