Clinical Overview

Bethanechol Chloride Tablets (25mg) is a synthetic choline ester direct-acting parasympathomimetic agent on the WHO Model List of Essential Medicines. It selectively stimulates muscarinic M3 and M2 receptors of the parasympathetic nervous system with negligible nicotinic activity and complete resistance to degradation by acetylcholinesterase. In the bladder, it stimulates detrusor muscle contraction, increases maximal detrusor pressure, reduces bladder capacity, and relaxes the trigone and external sphincter to initiate urination and completely empty the bladder. Indicated for acute non-obstructive post-operative and postpartum urinary retention, and neurogenic bladder atony with retention.

Usage Guidelines

Take orally 25 mg to 50 mg three to four times daily on an empty stomach: at least 1 hour before meals or 2 hours after meals with water to avoid nausea and vomiting. Dosage is titrated based on residual urine volume.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Abdominal cramps, diarrhea, nausea, increased salivation, flushing, sweating, urinary urgency. Rare/Severe: Bronchospasm, severe bradycardia, hypotension, and acute asthma exacerbation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Strictly contraindicated in mechanical urinary or gastrointestinal obstruction, bronchial asthma, active peptic ulcer disease, hyperthyroidism, epilepsy, Parkinson's disease, and severe bradycardia/hypotension. For oral use ONLY; NEVER administer parenterally (causes life-threatening cholinergic storm and cardiac arrest).
