Recombinant Human Tissue-type Plasminogen Activator (rt-PA / Alteplase) is a fibrin-specific thrombolytic biological enzyme on the WHO Model List of Essential Medicines. It binds selectively to fibrin in a thrombus and converts trapped plasminogen into active plasmin, which cleaves fibrin matrices and dissolves the occlusive blood clot, restoring organ perfusion. Indicated for acute ischemic stroke (within 4.5 hours of symptom onset), acute ST-elevation myocardial infarction (STEMI), and massive acute pulmonary embolism with hemodynamic instability.

Usage Guidelines
Administered strictly by critical care physicians or stroke neurologists in hospital intensive care/emergency settings via intravenous infusion after non-contrast head CT rules out intracranial hemorrhage. Requires continuous blood pressure and neurological monitoring.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Minor bleeding (puncture sites, gums, hematuria), epistaxis, ecchymosis. Rare/Severe: Fatal intracranial hemorrhage (ICH), massive gastrointestinal/retroperitoneal bleeding, angioedema of the tongue/lips, and anaphylaxis.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Absolute contraindications: Evidence of intracranial hemorrhage on CT, active internal bleeding, recent head trauma or stroke within 3 months, intracranial neoplasm, arteriovenous malformation, platelet count < 100,000, and uncontrolled severe hypertension (> 185/110 mmHg). Avoid invasive arterial punctures or anticoagulants for 24 hours post-infusion.
