Clinical Overview
Alendronate Sodium Tablets (70mg) is an oral nitrogen-containing bisphosphonate on the WHO Model List of Essential Medicines. It binds to bone hydroxyapatite and inhibits osteoclast-mediated bone resorption by selectively inhibiting farnesyl pyrophosphate synthase, increasing bone mineral density (BMD) and reducing vertebral and hip fracture incidence by ~50%. Indicated for treatment and prevention of postmenopausal osteoporosis, osteoporosis in men, and glucocorticoid-induced osteoporosis.

Usage Guidelines
CRITICAL ADMINISTRATION INSTRUCTIONS: Take orally 70 mg ONCE WEEKLY immediately upon arising in the morning, at least 30 minutes before the first food, beverage (other than plain water), or medication of the day. Swallow whole with a full glass (at least 200-250 mL) of PLAIN WATER ONLY (no mineral water, juice, tea, or coffee). The patient MUST REMAIN FULLY UPRIGHT (sitting or standing) for at least 30 minutes following administration and until the first meal, to prevent esophageal ulceration.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Abdominal pain, dyspepsia, acid regurgitation, nausea, musculoskeletal bone/joint pain, diarrhea or constipation. Rare/Severe: Severe esophageal ulceration, erosions, and strictures; Osteonecrosis of the Jaw (ONJ); and atypical subtrochanteric femur fractures with long-term use (>5 years).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Severe esophageal mucosal irritation and ulceration; failure to remain upright for 30 minutes or swallow with plain water causes severe esophagitis. Strictly contraindicated in esophageal abnormalities that delay emptying (stricture, achalasia), inability to stand or sit upright for 30 minutes, hypocalcemia, and severe renal impairment (eGFR < 35 mL/min). Consider "bisphosphonate holiday" after 3 to 5 years.
