Clinical Overview

Iron Sucrose Injection (20mg/ml elemental iron, 100mg/5ml ampoule) is a sterile parenteral polynuclear iron(III)-hydroxide sucrose complex on the WHO Model List of Essential Medicines. It exhibits high thermodynamic stability and low toxicity, delivering bioavailable iron directly to transferrin and bone marrow erythroblasts with a very low incidence of anaphylaxis compared to iron dextran. Indicated for treatment of iron deficiency anemia in hemodialysis-dependent and non-dialysis chronic kidney disease, severe gestational anemia, inflammatory bowel disease, and surgical blood management.

Usage Guidelines

Administer intravenously only: either by slow IV injection (undiluted at a maximum rate of 20 mg elemental iron per minute, i.e., 5 minutes per 100 mg ampoule) or by IV infusion (100 mg diluted in maximum 100 mL 0.9% NaCl over at least 15 minutes; 200 mg over at least 30 minutes). Monitor patient for at least 30 minutes post-dose. NEVER inject IM.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Hypotension, nausea, dizziness, dysgeusia (metallic taste), injection site burning, headache. Rare/Severe: Severe anaphylactoid reactions, circulatory collapse with rapid injection, and tissue necrosis upon extravasation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Administer slowly to avoid rapid free iron saturation leading to acute hypotension and peripheral vasodilation. Monitor vital signs during infusion. Extravasation causes brown staining of skin. Contraindicated in iron overload (hemochromatosis) and non-iron deficiency anemias.
