Sofosbuvir (400mg) and Velpatasvir (100mg) Tablets is a fixed-dose, pan-genotypic direct-acting antiviral (DAA) regimen on the WHO Model List of Essential Medicines. Sofosbuvir is a nucleotide prodrug that inhibits HCV NS5B RNA-dependent RNA polymerase, causing chain termination. Velpatasvir is a potent inhibitor of HCV NS5A protein, preventing viral RNA replication and virion assembly. Achieves >95-98% sustained virologic response (SVR12/cure) across all HCV genotypes (1 through 6). Indicated for the treatment of adult and pediatric patients with chronic hepatitis C virus (HCV) infection with or without compensated cirrhosis, and with ribavirin for decompensated cirrhosis.

Usage Guidelines
Take orally 1 tablet once daily with or without food for 12 weeks. Swallow whole with water; do not crush or chew. Adherence without missed doses is paramount to achieve viral cure.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Headache, fatigue, nausea, insomnia, asthenia. Rare/Severe: Severe symptomatic bradycardia when co-administered with amiodarone, and hepatitis B reactivation in co-infected patients.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Risk of Hepatitis B Virus (HBV) reactivation in patients co-infected with HCV/HBV; screen for HBV before initiation. Fatal bradycardia risk when co-administered with amiodarone; concomitant use is strictly contraindicated. Do not take with strong P-gp inducers (rifampin, St. John's wort). Space antacids by 4 hours.
