Clinical Overview

Mesalazine / Mesalamine (400mg, 500mg, 800mg, 1200mg Delayed Release / Prolonged Release Tablets) is an oral 5-aminosalicylic acid (5-ASA) anti-inflammatory agent on the WHO Model List of Essential Medicines. Formulated with pH-dependent (pH >= 7) enteric coatings or multi-matrix (MMX) core systems to bypass gastric degradation and deliver high local 5-ASA concentrations specifically to the terminal ileum and colon. Locally inhibits 5-lipoxygenase, reduces leukotriene B4 and pro-inflammatory prostaglandin synthesis, and scavenges free radicals without systemic corticosteroid toxicity. Indicated for induction and maintenance of clinical and endoscopic remission in mild-to-moderate ulcerative colitis.

Usage Guidelines

Take orally with meals with a full glass of water. Active flare: 2.4 g to 4.8 g daily in divided doses (or 2.4 g to 4.8 g once daily with MMX formulations). Remission maintenance: 1.2 g to 2.4 g once daily. Swallow tablets whole; do NOT crush, split, or chew the specialized coating.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Headache, nausea, diarrhea, abdominal pain, flatulence, dyspepsia, rash. Rare/Severe: Acute intolerance syndrome (cramping, bloody diarrhea mimicking UC flare), interstitial nephritis and renal impairment, pancreatitis, and pericarditis/myocarditis.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Monitor renal function (serum creatinine, urinalysis) prior to therapy, every 3 months for the first year, and annually thereafter. Contraindicated in severe renal or hepatic impairment and salicylate hypersensitivity. If paradoxical bloody diarrhea occurs, discontinue immediately.
