Clinical Overview

Metoclopramide Injection (5mg/ml) is a parenteral dopamine D2 receptor antagonist and 5-HT4 receptor agonist on the WHO Model List of Essential Medicines. It blocks central dopamine receptors in the CTZ and peripheral receptors in the gut, augmenting gastric antral contractions, relaxing the pyloric sphincter, and accelerating gastric emptying. Indicated for prevention and treatment of postoperative nausea and vomiting, acute chemotherapy-induced emesis, acute migraine nausea, and facilitation of small bowel intubation or radiologic GI transit.

Usage Guidelines

Administer by slow intravenous injection over at least 3 minutes, or intramuscularly. Slow IV injection is critical to prevent intense anxiety, agitation, and restlessness. Maximum daily dose is 30 mg (or 0.5 mg/kg/day). Limit use to a maximum of 5 days.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Drowsiness, restlessness, fatigue, diarrhea, hypotension. Rare/Severe: Acute dystonic reactions (facial spasms, oculogyric crisis, torticollis), tardive dyskinesia (potentially irreversible with chronic use), neuroleptic malignant syndrome, and methemoglobinemia.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black Box Warning: Tardive dyskinesia risk increases with cumulative dose and duration; do not exceed 5 days of treatment. Contraindicated in pheochromocytoma, epilepsy, mechanical bowel obstruction, or GI perforation. Have diphenhydramine or benztropine available to treat acute dystonic reactions.
