Clinical Overview
Carboprost Tromethamine Injection 125 mcg is a synthetic 15-methyl analogue of Prostaglandin F2-alpha (PGF2a). It produces sustained, intense myometrial contractions and local vasoconstriction to achieve mechanical hemostasis at the placental bed. Indicated for the control and refractory treatment of severe postpartum hemorrhage (PPH) due to uterine atony that has failed to respond to oxytocin, methylergometrine, or uterine massage, and for mid-trimester abortion.

Usage Guidelines
For Deep Intramuscular (IM) injection ONLY: 125 mcg to 250 mcg deep IM into the gluteal muscle or anterolateral thigh, administered by an obstetrician. In severe PPH, may repeat at 15 to 90 minute intervals as clinically necessary (maximum cumulative dose: 2 mg). NEVER administer intravenously (severe risk of bronchospasm, hypertension, and pulmonary edema).
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Diarrhea (in >60% of patients due to intestinal smooth muscle stimulation), vomiting, nausea, flushing, transient pyrexia/chills, shivering, and headache. Rare/Severe: Severe acute bronchospasm, acute hypertension, pulmonary edema, cardiac arrest, and uterine rupture.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Absolute Contraindication: Acute active pelvic inflammatory disease, active severe pulmonary/cardiac disease, and severe asthma (potent bronchoconstrictor). Use with extreme caution in patients with hepatic or renal impairment, history of bronchial asthma, epilepsy, or cardiovascular disease.
