Clinical Overview
Ranolazine is a piperazine derivative late sodium current (I_Na,late) inhibitor. In ischemic cardiomyocytes, pathological enhancement of late sodium influx leads to intracellular sodium accumulation, triggering the reverse mode of the Na+/Ca2+ exchanger and causing toxic intracellular calcium overload, diastolic left ventricular wall tension, and microvascular compression. Ranolazine selectively inhibits I_Na,late, attenuating calcium overload and improving diastolic myocardial perfusion without lowering heart rate or blood pressure. Indicated for the treatment of chronic stable angina pectoris.

Usage Guidelines
Swallow extended-release tablets whole with water, twice daily, with or without meals. Do not crush, break, or chew tablets. Complete regular medical evaluations.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Dizziness, headache, constipation, nausea, asthenia. Rare/Severe: Dose-dependent QTc interval prolongation, torsades de pointes, syncope, and acute renal failure in severe renal impairment.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Prolongs the QTc interval; caution in patients with congenital long QT syndrome or taking other QTc-prolonging drugs. Contraindicated in clinically significant hepatic cirrhosis and concomitant administration of strong CYP3A4 inhibitors (ketoconazole, clarithromycin) or inducers (rifampicin, carbamazepine).
