Clinical Overview
Doxorubicin Hydrochloride Injection (50mg / 2mg/ml) is a prototypical anthracycline cytotoxic antibiotic on the WHO Model List of Essential Medicines. It exerts anti-tumor effects through three mechanisms: DNA intercalation, topoisomerase II inhibition leading to double-strand DNA breaks, and generation of iron-mediated reactive oxygen free radicals that damage cell membranes and cellular organelles. Indicated for breast carcinoma, Hodgkin and non-Hodgkin lymphomas, acute lymphoblastic/myeloblastic leukemias, soft tissue sarcomas, osteosarcoma, and gastric cancer.

Usage Guidelines
Administer intravenously by slow IV push (over 3 to 10 minutes) or short IV infusion into the tubing of a freely flowing IV infusion of 0.9% NaCl or 5% Dextrose. Ensure secure vascular access; NEVER administer IM or subcutaneously. Cumulative lifetime dose must NOT exceed 450 to 550 mg/m² to prevent irreversible congestive heart failure.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Alopecia (universal, reversible), acute nausea and vomiting, dose-limiting myelosuppression, stomatitis/mucositis, harmless reddish-orange discoloration of urine for 1-2 days post-infusion. Rare/Severe: Irreversible dilated cardiomyopathy / congestive heart failure, severe local extravasation necrosis, and radiation recall reaction.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Cumulative lifetime dose-dependent cardiotoxicity; perform baseline and periodic echocardiography or MUGA scans (LVEF monitoring). Black Box Warning: Potent vesicant; extravasation causes deep tissue ulceration and necrosis (dexrazoxane is the specific antidote). Black Box Warning: Severe myelosuppression.
