Clinical Overview
Cisplatin Injection (10mg/10ml, 50mg/50ml) is a first-generation platinum coordination complex antineoplastic agent on the WHO Model List of Essential Medicines. It forms intrastrand and interstrand crosslinks with DNA purine bases (predominantly guanine-guanine), distorting the DNA helix, halting replication and transcription, and triggering p53-dependent apoptotic cell death. Indicated for treatment of metastatic testicular cancer, advanced ovarian cancer, advanced bladder cancer, squamous cell carcinoma of the head and neck, non-small cell lung cancer, and cervical cancer.

Usage Guidelines
Administer intravenously only as an IV infusion (usually 20 to 100 mg/m² per cycle) over 1 to 2 hours. Mandatory pre-hydration and post-hydration with at least 1 to 2 liters of normal saline containing mannitol is required to induce vigorous diuresis and prevent severe nephrotoxicity. Premedicate with 5-HT3 antagonists and dexamethasone for high emetogenic potential.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Severe acute and delayed nausea and vomiting (emetogenic level 5, >90%), cumulative nephrotoxicity (elevated creatinine/BUN), ototoxicity (bilateral high-frequency tinnitus/hearing loss), peripheral sensory neuropathy. Rare/Severe: Acute renal tubular necrosis, severe myelosuppression, and anaphylactoid reactions.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Cumulative, dose-dependent nephrotoxicity; ensure vigorous hydration and diuresis. Black Box Warning: Ototoxicity and peripheral neuropathy. Monitor serum creatinine, BUN, creatinine clearance, magnesium, and audiometry prior to each cycle. Contraindicated in pre-existing renal impairment, hearing impairment, or bone marrow depression.
