Clinical Overview
Meropenem is an intravenous synthetic carbapenem bactericidal antibiotic with broad-spectrum activity against Gram-positive, Gram-negative, and anaerobic pathogens. It acts by inhibiting bacterial cell wall synthesis via high-affinity binding to essential PBPs (PBP 2, 3, and 4). Has low epileptogenic potential compared to imipenem. Indicated for complicated intra-abdominal infections, complicated skin and soft tissue infections, bacterial meningitis in paediatric and adult patients, nosocomial pneumonia, and febrile neutropenia.

Usage Guidelines
Administered solely by healthcare professionals by intravenous bolus injection over 3-5 minutes or IV infusion over 15 to 30 minutes after reconstitution with sterile diluent. In severe hospital infections, prolonged infusions over 3 hours are used to maximize pharmacodynamic time above MIC.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Diarrhea, nausea, vomiting, headache, rash, thrombophlebitis at injection site, transient elevation of transaminases. Rare/Severe: Seizures and encephalopathy, anaphylactic shock, pseudomembranous colitis, and severe cutaneous adverse reactions.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Contraindicated in carbapenem allergy. Absolute precaution: Concomitant administration with valproic acid causes rapid, profound drop in serum valproate concentrations, leading to loss of seizure control; combination should generally be avoided. Adjust dose in renal impairment.
