Clinical Overview
Zoledronic Acid Injection (4mg/100ml, 5mg/100ml) is a third-generation nitrogen-containing heterocyclic bisphosphonate on the WHO Model List of Essential Medicines. It binds avidly to hydroxyapatite crystals in mineralized bone matrix and is internalized by active osteoclasts, where it potently inhibits farnesyl pyrophosphate (FPP) synthase in the mevalonate pathway. This prevents prenylation of small GTPases (Rho, Rac, Rab), causing loss of osteoclast ruffled borders, cytoskeletal collapse, and osteoclast apoptosis, powerfully arresting bone resorption. Indicated for treatment of postmenopausal osteoporosis and osteoporosis in men (5 mg annual IV infusion), Paget's disease of bone, hypercalcemia of malignancy (HCM), and prevention of skeletal-related events in patients with bone metastases from solid tumors or multiple myeloma (4 mg every 3-4 weeks).

Usage Guidelines
Administer INTRAVENOUSLY ONLY as a slow IV infusion over at least 15 minutes. Infusion time must NEVER be less than 15 minutes to avoid acute renal failure. Patients MUST be adequately hydrated (at least 500 mL normal saline prior to and after infusion) and have normal serum creatinine/calcium. Premedicate with paracetamol to reduce acute phase reaction. Daily calcium (1000-1200 mg) and vitamin D (800-1000 IU) supplementation is mandatory.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Acute phase reaction (flu-like symptoms: fever, myalgia, bone pain, arthralgia, headache occurring within 3 days in ~30-40% of patients), hypocalcemia, nausea, fatigue. Rare/Severe: Acute renal tubular necrosis / renal failure, Osteonecrosis of the Jaw (ONJ), atypical subtrochanteric femoral fractures, and severe hypocalcemia.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Acute kidney injury and renal failure; strictly contraindicated in severe renal impairment (eGFR < 35 mL/min for osteoporosis, or CrCl < 30 mL/min). Infuse over >=15 minutes with adequate pre-hydration. Black Box Warning: Osteonecrosis of the Jaw (ONJ); perform thorough dental examination and complete invasive dental procedures prior to therapy. Correct pre-existing hypocalcemia before dosing.
