Clinical Overview

Lenalidomide Capsules (5mg, 10mg, 25mg) is a potent second-generation 4-amino-glutamyl analogue immunomodulatory drug (IMiD) on the WHO Model List of Essential Medicines. It binds directly to cereblon (CRBN), the substrate recognition component of the cullin-RING E3 ubiquitin ligase complex CRL4-CRBN, inducing selective ubiquitination and proteasomal degradation of lymphoid transcription factors IKZF1 (Ikaros) and IKZF3 (Aiolos). This causes direct multiple myeloma cell apoptosis, downregulates IRF4 and MYC, stimulates IL-2 production to activate NK and T-cells, and inhibits pro-angiogenic factors (VEGF, bFGF). Indicated in combination with dexamethasone for multiple myeloma, myelodysplastic syndrome (MDS) associated with deletion 5q, and relapsed mantle cell lymphoma.

Usage Guidelines

Take orally once daily at approximately the same time each day with water, with or without food. Multiple myeloma: standard regimen is 25 mg daily on days 1 to 21 of repeated 28-day cycles. Swallow capsules whole; do NOT open, break, or chew. MANDATORY: Thromboprophylaxis (aspirin 75-100 mg daily for low risk, or therapeutic LMWH/DOAC for high risk) is required.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dose-limiting myelosuppression (neutropenia, thrombocytopenia), fatigue, muscle cramps, diarrhea, constipation, peripheral edema, pruritus/rash. Rare/Severe: Deep vein thrombosis (DVT) and pulmonary embolism (PE), severe teratogenicity / phocomelia, secondary primary malignancies, and Stevens-Johnson syndrome.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black Box Warning: Severe, life-threatening teratogenicity (severe birth defects or fetal death); mandatory pregnancy prevention program (REMS), negative pregnancy test prior to each monthly prescription, and dual reliable contraception. Black Box Warning: Venous and arterial thromboembolism; mandatory thromboprophylaxis. Black Box Warning: Hematologic toxicity (neutropenia/thrombocytopenia). Dose reduction required in renal impairment.
