🧴 Clinical Application
Take orally once daily at approximately the same time each day with water, with or without food. Multiple myeloma: standard regimen is 25 mg daily on days 1 to 21 of repeated 28-day cycles. Swallow capsules whole; do NOT open, break, or chew. MANDATORY: Thromboprophylaxis (aspirin 75-100 mg daily for low risk, or therapeutic LMWH/DOAC for high risk) is required.
⚠️ Side Effects
Common: Dose-limiting myelosuppression (neutropenia, thrombocytopenia), fatigue, muscle cramps, diarrhea, constipation, peripheral edema, pruritus/rash. Rare/Severe: Deep vein thrombosis (DVT) and pulmonary embolism (PE), severe teratogenicity / phocomelia, secondary primary malignancies, and Stevens-Johnson syndrome.

Black Box Warning: Severe, life-threatening teratogenicity (severe birth defects or fetal death); mandatory pregnancy prevention program (REMS), negative pregnancy test prior to each monthly prescription, and dual reliable contraception. Black Box Warning: Venous and arterial thromboembolism; mandatory thromboprophylaxis. Black Box Warning: Hematologic toxicity (neutropenia/thrombocytopenia). Dose reduction required in renal impairment.

This causes direct multiple myeloma cell apoptosis, downregulates IRF4 and MYC, stimulates IL-2 production to activate NK and T-cells, and inhibits pro-angiogenic factors (VEGF, bFGF). Indicated in combination with dexamethasone for multiple myeloma, myelodysplastic syndrome (MDS) associated with deletion 5q, and relapsed mantle cell lymphoma.
