Clinical Overview
Bicalutamide Tablets (50mg) is an oral non-steroidal antiandrogen on the WHO Model List of Essential Medicines. It competitively antagonizes androgen binding to nuclear androgen receptors (AR) in the prostate gland and peripheral target tissues, preventing DHT-induced gene expression and cellular proliferation without displaying progestational or glucocorticoid agonist activity. Indicated in combination with a luteinizing hormone-releasing hormone (LHRH / GnRH) analogue for treatment of stage D2 metastatic prostate carcinoma (complete androgen blockade) and to prevent initial tumor flare upon LHRH agonist initiation.

Usage Guidelines
Take orally 50 mg once daily at the same time each day (with or without food) with a glass of water, in combination with an LHRH analogue. Initiate bicalutamide at least 3 days prior to the first LHRH agonist injection to block symptomatic "testosterone flare".
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Hot flashes, gynecomastia, breast pain/tenderness, asthenia, back pain, constipation, peripheral edema. Rare/Severe: Severe drug-induced hepatotoxicity / hepatic failure, interstitial lung disease, and cardiac failure.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Monitor liver function tests (ALT, AST, bilirubin) prior to therapy, every month for the first 4 months, and periodically thereafter; discontinue immediately if transaminases exceed 2× ULN with jaundice. Discontinue if unexplained pulmonary symptoms (dyspnea, cough) occur.
