Clinical Overview
Pilocarpine Hydrochloride (2% w/v eye drops) is a direct parasympathomimetic (cholinergic) miotic agent on the WHO Model List of Essential Medicines. It stimulates muscarinic receptors on the pupillary sphincter muscle (causing miosis) and the ciliary muscle (causing contraction that pulls the scleral spur and opens the trabecular meshwork), facilitating aqueous outflow. Indicated for acute angle-closure glaucoma emergency management, chronic open-angle glaucoma, and reversal of mydriasis.

Usage Guidelines
Instill one drop into the affected eye(s) up to 3-4 times daily as prescribed. In acute angle closure, administered under direct ophthalmologist protocol alongside systemic agents.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Ciliary spasm (brow ache, temporal headache), induced myopia (nearsightedness), reduced visual acuity in poor lighting/night blindness due to miosis, eye irritation. Rare/Severe: Retinal detachment (especially in myopic patients with pre-existing lattice degeneration) and systemic cholinergic symptoms.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Poor night vision: warn patients against driving in dark environments. Dilated fundus examination is recommended prior to initiation to rule out retinal tears in myopes. Contraindicated where pupillary constriction is undesirable (acute uveitis).
