Clinical Overview

Ipratropium Bromide Inhaler (20mcg) and Respirator Solution (250mcg per ml) is an inhaled synthetic quaternary ammonium anticholinergic bronchodilator on the WHO Model List of Essential Medicines. It competitively antagonizes muscarinic M1, M2, and M3 receptors on bronchial smooth muscle, blocking acetylcholine released from parasympathetic vagal efferents. Because of its quaternary structure, it does not cross mucosal membranes or the blood-brain barrier, exerting localized bronchodilation with minimal systemic anticholinergic adverse effects. Indicated for maintenance bronchodilation and acute relief of bronchospasm in Chronic Obstructive Pulmonary Disease (COPD) and adjunctive management of severe acute asthma.

Usage Guidelines

Inhaler: Inhale 2 puffs four times daily at regular intervals (maximum 12 puffs daily). Respirator solution: Administer 250 to 500 mcg diluted in 0.9% NaCl via a jet nebulizer over 10-15 minutes. Protect eyes during administration.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dry mouth (xerostomia), bitter taste, throat irritation, cough, headache, dizziness, nausea. Rare/Severe: Acute angle-closure glaucoma (if sprayed in eyes), urinary retention (in BPH), and paradoxical bronchospasm.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Do NOT allow mist or spray to enter the eyes (can precipitate acute angle-closure glaucoma and pupillary dilation). Use with caution in patients with narrow-angle glaucoma or bladder outlet obstruction.
