Clinical Overview

Perampanel is a first-in-class, highly selective, non-competitive antagonist at postsynaptic alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) glutamate receptors on excitatory neurons. By targeting the primary mediator of fast excitatory synaptic transmission in the brain, it halts seizure propagation. Indicated as adjunctive therapy for partial-onset seizures and primary generalized tonic-clonic seizures.

Usage Guidelines

Take orally once daily immediately before bedtime with water, with or without food. Bedtime dosing minimizes daytime dizziness and somnolence. Titrate slowly at minimum 1-to-2 week intervals.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dizziness, somnolence, fatigue, irritability, ataxia, vertigo, weight gain, nausea. Rare/Severe: Boxed warning: Serious psychiatric and behavioral reactions (severe aggression, hostility, anger, homicidal thoughts and threats), falls, and severe hypersensitivity.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black box warning: Serious neuropsychiatric events including hostility, severe aggression, and homicidal ideation; monitor patients and instruct families to report sudden personality or behavioral changes immediately. Dose-dependent risk of dizziness and severe falls.
