Ibrutinib Capsules (140mg) is a first-in-class oral irreversible inhibitor of Bruton's Tyrosine Kinase (BTK) on the WHO Model List of Essential Medicines. It forms a covalent bond with cysteine 481 in the BTK active site, permanently disabling B-cell antigen receptor (BCR) signaling and halting malignant B-cell proliferation, survival, and homing to lymphatic niches. Indicated for Chronic Lymphocytic Leukemia (CLL) / Small Lymphocytic Lymphoma (SLL), Mantle Cell Lymphoma (MCL), Waldenstrom's Macroglobulinemia, and chronic Graft-versus-Host Disease (cGVHD).

Usage Guidelines
Take orally once daily (420 mg for CLL, 560 mg for MCL) with a full glass of water at approximately the same time each day, with or without food. Swallow capsules whole; do not open or dissolve. Transient redistribution lymphocytosis is expected during initial weeks and does not signify disease progression.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Diarrhea, fatigue, musculoskeletal pain, bruising/ecchymosis, upper respiratory infection, nausea, rash. Rare/Severe: Atrial fibrillation/flutter (up to 6-9%), ventricular arrhythmias, severe bleeding/major hemorrhage (inhibits collagen-induced platelet aggregation), hypertension, and opportunistic infections.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Bleeding risk: avoid concomitant warfarin; withhold ibrutinib 3 to 7 days before and after surgical procedures. Monitor for cardiac arrhythmias (atrial fibrillation). Substrate of CYP3A4; avoid grapefruits, Seville oranges, and strong CYP3A4 inhibitors.
