Clinical Overview

Rivastigmine is a pseudo-irreversible carbamate dual inhibitor of both acetylcholinesterase (AChE) and butyrylcholinesterase (BuChE) on the WHO Model List of Essential Medicines. It facilitates cholinergic transmission by delaying acetylcholine degradation, with dual enzyme inhibition providing sustained efficacy in advanced disease where BuChE activity increases. Indicated for mild-to-moderate dementia of the Alzheimer's type and mild-to-moderate dementia associated with Parkinson's disease.

Usage Guidelines

Apply one transdermal patch once daily to clean, dry, hairless, intact skin on the upper or lower back, upper arm, or chest. Rotate application sites daily; do not apply a new patch to the exact same skin site for at least 14 days. Remove the previous patch before applying a new one. Do not cut the patch.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Application site erythema, pruritus, mild nausea, weight loss, anorexia. Rare/Severe: Allergic contact dermatitis, severe bradycardia, syncopal episodes, and dehydration.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Transdermal patch reduces gastrointestinal side effects compared to oral capsules; ensure only one patch is worn at a time to prevent life-threatening cholinergic overdose. Vagotonic effects can cause bradycardia. Caution in asthma, COPD, and peptic ulcer disease.
