Clinical Overview
Eltrombopag Tablets (25mg, 50mg) is an oral, non-peptide thrombopoietin (TPO) receptor agonist on the WHO Model List of Essential Medicines. It interacts with the transmembrane domain of the human TPO receptor (c-Mpl) on megakaryocytes and hematopoietic stem cells, initiating JAK/STAT and MAPK signaling pathways that stimulate megakaryocyte proliferation and differentiation, elevating platelet counts. Indicated for chronic immune thrombocytopenia (ITP) in patients who have had an insufficient response to corticosteroids or immunoglobulins, severe aplastic anemia, and thrombocytopenia in patients with chronic hepatitis C to allow interferon therapy.

Usage Guidelines
Take orally once daily on an EMPTY STOMACH: at least 1 hour before or 2 hours after food, and at least 2 hours before or 4 hours after mineral-containing foods or supplements (dairy products, antacids, iron, calcium, magnesium), as polyvalent cations chelate eltrombopag and reduce absorption by >70%. Titrate dose to maintain platelet count >= 50,000/microL (do not titrate to normal platelet counts).
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Nausea, diarrhea, headache, upper respiratory tract infection, abnormal liver function tests, fatigue. Rare/Severe: Severe hepatotoxicity / hepatic failure, thromboembolic complications (portal vein thrombosis), and bone marrow reticulin fiber deposition.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Risk of severe hepatotoxicity / liver decompensation; monitor serum transaminases and bilirubin every 2 weeks during titration, then monthly. High risk of thromboembolism if platelets rise >200,000-400,000/microL; adjust dose strictly to achieve minimum platelet count needed to avoid major bleeding. Must be taken apart from dairy products and polyvalent cations.
