Clinical Overview
Topiramate is a sulfamate-substituted monosaccharide antiepileptic with multiple mechanisms of action. It blocks voltage-dependent sodium channels, potentiates GABA-mediated inhibitory currents at non-benzodiazepine sites, antagonizes AMPA/kainate glutamate receptors, and weakly inhibits carbonic anhydrase isozymes CA-II and CA-IV. Indicated for partial-onset seizures, primary generalized tonic-clonic seizures, Lennox-Gastaut syndrome, and migraine prophylaxis in adults and adolescents.

Usage Guidelines
Swallow tablets whole with water; do not break or chew due to an intense bitter taste. Can be taken with or without food. Maintain abundant daily fluid intake (at least 2-3 liters) to prevent kidney stone formation. Titrate slowly.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Paresthesia (tingling in fingers/toes), cognitive slowing, word-finding difficulty, memory impairment, anorexia, weight loss, somnolence, fatigue. Rare/Severe: Acute secondary angle-closure glaucoma and acute myopia, oligohidrosis (reduced sweating) and hyperthermia, metabolic acidosis (via carbonic anhydrase inhibition), nephrolithiasis (kidney stones), and teratogenicity (cleft lip/palate).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Patients experiencing sudden eye pain, redness, or blurred vision must stop immediately and seek emergency ophthalmological care (risk of permanent blindness from angle-closure glaucoma). Ensure high fluid intake to prevent kidney stones. Contraindicated in pregnancy for migraine prophylaxis due to oral cleft risk.
