Clinical Overview

S-Adenosyl L-Methionine (SAMe) Tablets (200mg, 400mg) is an enteric-coated biological sulfonium compound and primary biological methyl donor on the PMBI formulary. It is a vital precursor in transmethylation, transsulfuration (generating glutathione, taurine, and sulfate), and polyamine pathways in hepatocytes. In cholestasis, SAMe restores hepatocyte membrane fluidity by methylating phospholipids and enhances glutathione-mediated detoxification. Indicated for intrahepatic cholestasis in pre-cirrhotic and cirrhotic states, intrahepatic cholestasis of pregnancy (IHCP), and alcoholic/toxic liver disease.

Usage Guidelines

Take orally 400 mg to 800 mg twice daily, at least 30 minutes before meals with water on an empty stomach. Swallow the enteric-coated tablet whole; do NOT crush, split, or chew, as gastric acid degrades unprotected SAMe.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Nausea, abdominal discomfort, diarrhea, insomnia, anxiety. Rare/Severe: Mania or hypomania in patients with bipolar disorder, and serotonin syndrome when combined with serotonergic antidepressants.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Take on an empty stomach for proper absorption. Contraindicated in patients with bipolar disorder (risk of precipitating manic episodes). Avoid concomitant use with MAOIs, SSRIs, or TCAs due to serotonin syndrome risk.
