Dextromethorphan Hydrobromide Syrup (13.5mg/5ml), Lozenges (5mg; alone or with Phenylephrine and Chlorpheniramine) is an oral centrally acting non-opioid antitussive on the WHO Model List of Essential Medicines. It acts as an agonist at sigma-1 receptors and a non-competitive antagonist at NMDA glutamate receptors in the medullary cough center of the brainstem, raising the threshold for coughing without suppressing respiratory drive, inhibiting mucociliary clearance, or causing opioid dependence. Indicated for temporary relief of dry, hacking, non-productive cough caused by minor throat and bronchial irritation from the common cold or inhaled irritants.

Usage Guidelines
Adults: Take 10 to 20 mg orally every 4 hours, or 30 mg every 6 to 8 hours with water as needed (maximum 120 mg daily). Lozenges: Dissolve slowly in the mouth without chewing. Do NOT use for chronic productive wet cough.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Drowsiness, dizziness, nausea, vomiting, abdominal discomfort. Rare/Severe: Serotonin syndrome (when combined with SSRIs/MAOIs), and respiratory depression with massive supratherapeutic recreational doses.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Over-The-Counter antitussive. Strictly contraindicated with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of stopping (risk of fatal Serotonin Syndrome). Do NOT use to suppress a productive (wet) cough (suppression of productive cough causes mucus retention and pneumonia). Avoid alcohol.
