Clinical Overview

Sevelamer Carbonate / Hydrochloride Tablets (400mg, 800mg) is a non-calcium, non-aluminum, non-absorbable cross-linked polymeric phosphate binder on the WHO Model List framework. In the gastrointestinal tract, its multiple amine groups become partially protonated at physiological intestinal pH, binding dietary phosphate through ionic and hydrogen bonding. The bound phosphate is excreted in the feces, decreasing intestinal phosphate absorption without releasing calcium or aluminum into the bloodstream. This prevents hypercalcemia, calciphylaxis, and metastatic vascular calcification. Indicated for the control of hyperphosphatemia in adult and pediatric patients with Chronic Kidney Disease (CKD) on hemodialysis or peritoneal dialysis, or non-dialysis CKD with serum phosphorus >= 5.5 mg/dL.

Usage Guidelines

Take orally with meals three times daily with a full glass of water. Typical starting dose is 800 mg to 1600 mg with each meal, titrated by 400-800 mg per meal every 2 weeks based on serum phosphorus levels (target 3.5-5.5 mg/dL). Swallow the tablet whole; do NOT chew, crush, or break, as the polymer expands in water.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Nausea, vomiting, diarrhea, dyspepsia, abdominal pain, constipation, flatulence. Rare/Severe: Severe bowel obstruction, ileus, fecal impaction, and intestinal perforation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Must be taken WITH meals to bind ingested dietary phosphate; taking on an empty stomach renders the drug ineffective. Contraindicated in bowel obstruction, ileus, and severe fecal impaction. Space other oral medications (ciprofloxacin, levothyroxine, mycophenolate) at least 1 hour before or 3 hours after sevelamer to avoid binding.
