Clinical Overview

Vildagliptin (50 mg, 100 mg SR) is a cyanopyrrolidine dipeptidyl peptidase-4 (DPP-4) inhibitor. It binds covalently to the catalytic site of DPP-4 with slow substrate-like kinetics, slowing enzyme dissociation and providing sustained incretin (GLP-1 and GIP) stabilization. This increases insulin secretion and suppresses inappropriate glucagon secretion in a glucose-dependent manner, eliminating intrinsic hypoglycemia risk. Indicated for the treatment of type 2 diabetes mellitus.

Usage Guidelines

Take orally with water, with or without food. Standard dose is 50 mg twice daily (morning and evening) or 100 mg SR once daily. When added to a sulfonylurea, a 50 mg once-daily morning dose is used to prevent hypoglycemia.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Headache, tremor, dizziness, nausea, nasopharyngitis, mild peripheral edema. Rare/Severe: Hepatic dysfunction (transaminitis, rare acute hepatitis), acute pancreatitis, and bullous pemphigoid.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Liver function tests (ALT/AST) must be conducted prior to initiation, at 3-month intervals during the first year, and periodically thereafter; do not initiate if ALT/AST > 3x ULN. Reduce dose to 50 mg once daily in moderate-to-severe renal impairment. Discontinue if persistent abdominal pain develops.
